Healthcare Forms
Everyone with health insurance should receive a Form 1095 around tax season. If you have health insurance with your employer then you should receive a Form 1095-B and Form 1095-C. If you have health insurance through the marketplace then you should receive a Form 1095-A. Please bring the Form 1095-A that you receive with your other tax documents; this form is needed to complete your return. Forms 1095-B and 1095-C are no longer required.
